Certificte of Good Conduct (شهادة حسن سيرة وسلوك)

Requirements:

1. Filled form. (not handwritten).
2. Your Palestinian passport and your Palestinian identity card.
3. Your status in Canada or Canadian identification (i.e.: PR Card, Canadian passport/citizenship, Refugee status ).
4. Two personal photos with white background 4*6 cm (photography studio, date picture was taken and name should be written on the back of one of the photos).
5. Fees: 35 CAD in form of a money order payable to the Palestinian General Delegation.
6. A prepaid registered return envelope must be included. (if sent by mail).

The application can be submitted:

• In person at the Palestinian General Delegation in Ottawa by scheduling an appointment by phone or email.
• By mail: first, Documents must be signed and notarized by notary public (virtual notarization via zoom or similar video calling softwares is not accepted, notary public must attest to the signature in persone), then mail the documents to “Palestinian General delegation”. A prepaid registered return envelope must be included with the documents.

Note: All forms and/or documents that come from Palestine must be authenticated by the Palestinian Ministry of Foreign Affairs.
